I think the next step would be to investigate whether there are any mouthpiece 'technicians' in your part of the world (is it actually the USA) and if so you could visit in person and get the job done exactly as you want it by 'trial and error'. Do bear in mind it might be quite expensive!
If not, then I am sure, as I said, that Paxman or Osmun could do it for you, also other firms such as Stork or Moosewood, but this would have to be by post unless you could visit these shops.
I strongly reccomend that you keep your present 18.5 rim and cup combination as a reference point, and have another one (or two?) adjusted. Maybe get one of the rims opened out to 19.00mm or 19.5mm and the other 20mm. What rim do you actually use? (eg. W, M etc)
Of course, the joining point of the top of the cup(s) would need adjusting as there would be an internal ledge.
There are other factors to think about as well: if your rim is nickel-silver, then OK, but if it's gold-plated it would need re-plating after the work is done. Ask the technician to make sure there is no sharp edge at the top of the rim where it has been opened out, on the other hand, if it is totally rounded this can feel too slippery. This part of any mouthpiece rim is a critical point in the player's feeling of comfort and security.
