I go to a lot of trouble to sterilise items such as these (and also, particularly, cups when I can get them).
Firstly I leave mouthpiece components in a solution of TCP (disinfectant)for about 30 minutes and then rinse them in boiling water. Even brand-new mouthpieces are probably not given this treatmant!
